Greece and Italy in the spring; Japan, Korea and Taiwan in the fall — the same eSIM used both times, different data plans each time.
A reload isn’t tied to last trip’s countries. You pay for the destinations on this trip rather than an entire region that you’re mostly not visiting. Adding a country mid-trip puts it on the same eSIM. Still one eSIM, still nothing to install.

No. The QR code installed the eSIM, and the eSIM is already on your phone. A reload adds data to what’s installed — there’s nothing to scan and nothing to set up again.
Yes. Add every country on the trip and they all go onto one plan, on the one eSIM. It makes no difference whether last trip was one country or five.
The one you choose. At checkout your eSIMs are listed by the names you gave them, and you select which one to reload.
Nothing, and it costs nothing. The eSIM stays installed for as long as you like. When you travel again you reload it — no reinstalling.
